2. Download and complete the following forms (link on the right) and include copies of the additional documents in the application packet.

Additional Documents

Immunization/Health Records
Copy of Birth Certificate, Social Security Card, Driver's License (if applicable)
If student is Catholic, copies of Baptismal, Reconciliation, First Eucharist, and Confirmation certificates
