Transfer Admission Applications are accepted year-round. Please follow the steps below and contact Blake Evans at bevans@holytrinitychs.org if you have any further questions. 

Please follow these steps:

1. Complete the Transfer Admission Application (link on the right)

2. Download and complete the following forms (link on the right) and include copies of the additional documents in the application packet.

Forms

• Release of Records
• English/Language Arts Teacher Recommendation
• Math/Science Teacher Recommendation

• Immunization/Health Records
• Copy of Birth Certificate, Social Security Card, Driver's License (if applicable)
• If student is Catholic, copies of Baptismal, Reconciliation, First Eucharist, and Confirmation certificates
